Contacts and Certifications

This section of the SELPA Local Plan is divided by areas A1 through A14 that provide contacts, dates, and other information: 

  • Area A1 indicates what Local Plan Sections will be submitted to CDE. For example, a SELPA may submit Section B: Governance and Administration, Section D: Annual Budget Plan, and Section E: Annual Service Plan. This is the area where a SELPA would indicate if there were any membership changes. 
  • Area A2 indicates the SELPA's unique 4-digit code issued by CDE. 
  • Area A3 provides the SELPA Administrator's contact information such as name and address. 
  • Area A4 provides the SELPA's Administrative Entity information. Each SELPA must have an Administrative Unit. 
  • Area A5 indicates if the SELPA collaborated with the Community Advisory Committee.
  • Area A6 provides the date that the Local Plan was submitted to the Community Advisory Committee. 
  • Area A7 provides the date that the Local Plan was submitted to the County Office of Education. 
  • Areas A8 & A9 provides the date that Public Hearing notice was posted, and the date the Public Hearing to approve the Local Plan occurred. This is for the Annual Budget Plan and the Annual Service Plan. 
  • Area A10 indicates the SELPA type (Single LEA SELPA, Multiple LEA SELPA, or COE Joined SELPA). 
  • Area A11 provides the fiscal years when the previously Local Plan sections were submitted to CDE. 
  • Area A12 provides information on the representatives that were involved in the development of all sections of the Local Plan. 
  • Area A13 indicates which certifications are being submitted (Certifications 1 through 5). 
  • Area A14 mandates that certifications must be electronically signed and included with the Local Plan.
